Thank you for the reminder, because I meant to ask this myself.
A web search seems to indicate that he resided in Maple Ridge district,
British Columbia (greater Vancouver).

Nick was a PA who worked on the series, and was well liked by everyone on the
show. He died in a car accident on the way to work while we were shooting TLU
part one. Everybody felt very strongly that he deserved notation here.
